Have a look at ReviewBoard (http://www.reviewboard.org/), and the ReviewBoard extension for Mercurial (http://mercurial.selenic.com/wiki/ReviewboardExtension).

Obviously this requires a server to be set up somewhere, which may not meet your needs if you have to work in a distributed fashion, but if that's not a problem I can heartily recommend it.
