[issue96] mq: make qrefresh take list of files

It would be very helpful if qrefresh took a list of files to look at.

When I'm working on a moderately complex patch series, I'll sometimes want to
edit two patches at a time, but if I try to edit both, qrefresh will only apply
my changes to the topmost patch.  If I could get it to omit some files, I could
refresh the topmost patch, pop it, refresh the underlying patch that the others
files should be part of, then push again.
