I'd recommend Roundup (http://roundup.sourceforge.net/), just try
the instant gratification demo included in the tarball. We have a
public test installation based on an older version (roundup 0.6)
running at https://intevation.de/roundup/testroundup/ where you can
try registering a user and such things.

It can not only be used to track bugs, but it is easily extensible
with pieces of Python code, one of the extension we use is for
tracking patches which should be integrated upstream.
