Push creates new heads without -f

I haven't yet been able to nail down the exact circumstances in which this happens, but we keep getting multiple heads in the same named branch appearing in our master repository even though nobody is using the -f option when pushing. At least once I've caught it in the act of allowing a push that creates new heads.
We're using Mercurial 1.3.1. Although now that I think of it our server is still on 1.2.1, not sure if that could be the problem.
Has anyone else experienced this? Is there a workaround?
