Rebase documentation issues

No new extension... the rebase extension is shipped with Mercurial and
its homepage is this page:

  http://mercurial.selenic.com/wiki/RebaseExtension

See 'hg help rebase' for the help -- I believe the RebaseProject and
RebasePlan wiki pages were used for planning and discussion of the
extension when it were built and I would not use them when seeking help
about an extension.
