Track patch across branches?

Is it possible to determine which branches a patch has been applied to? Let's
say I fix a bug in the v3.2 branch, then graft or transplant the fix to
other branches. Other than searching for the commit message, how can I tell
6 months later which branches the fix has been applied to?

FWIW, TFS calls this "Track Changeset". There's a good description at
http://cyb3r.net/software-development-tools/tracking-changesets-in-vs2010/.
