The pager extension works under Windows as of Hg 2.2.2, so that patch
shouldn't be necessary. FWIW, I'm able to use it under cmd.exe with
more.

The issue with the help command not being paged consistently is a
different issue (one that also affects Linux, OS X, etc). When running
"hg --help", internally, Mercurial doesn't invoke the help command in
the same way it would if you did "hg help". The pager extension
doesn't see that a specific command was run, so it doesn't page the
output. I talked with mpm recently about fixing this, and I think he
might have a work-in-progress patch lying around for it.
